How to Complete and Submit the Form
Eligibility: You must be at least21 years old and of sound mind.
Witnesses: Sign the form in the presence of two witnesses (one must be adoctor, and the other must be an adult aged 21 or older who does not stand to gain from your death - Our Clinic assistant can help to be your 2nd witness)
Same Date: All signatures (yours and both witnesses) must bear the exactsame date.
Submission: Mail or hand-deliver the completed form in a sealed envelopeto the Registrar of Advance Medical Directives at the Ministry of Health
