6 Monthly Examinations (6ME)

- No fasting/ Appointment required.

- Pls bring along MOM 6ME form and Physical work permit

- Tests required based on MOM 6ME form:

• VDRL, Pregnancy

• VDRL, Pregnancy, HIV

• VDRL, pregnancy, HIV + Chest Xray (xray form will be provided by clinic and patient directed to nearest Xray facility)

MMR Vaccination 2 doses, 1 month apart - If have young children at home that arent vaccinated against MMR.

Annal Influenza( FLU) Vaccination (Optional)
